Hazard
Hazard: The quick release devices can unexpectedly fail or break when locked in
position on the bicycle, causing the rider to lose control, which poses a
serious fall hazard.
Description and Models: This recall involves the SunRinglé Hollow Quick Release
Devices for bicycles and includes models Hollow CroMo Front 100mm, Hollow CroMo
Rear 130mm, and Hollow CroMo Rear 135mm. These devices have hollow skewers. They
were sold individually or with SunRinglé front and rear wheels with model names
Black Flag, Disc o Flea, Charger, STR8 Track, Accelerator R3.00C and Accelerator
T3.0C. “Ringlé” is stamped on the lever. Quick release devices with a solid
skewer are not included in this recall.

Paintball Gun Adapters Recalled by RAP4 Due to Impact and Laceration Hazards
Hazard: The remote line adapter can burst when over tightened, posing serious
impact and laceration hazards to consumers.
Description: The remote line adapter is used to connect a paintball gun to a
pressurized CO2 tank. The adapter is a black cylinder and is attached to a black
remote line cord. “RAP4” and the “SN” (serial number) are printed on the

RadioShack Recalls Power Supplies Due to Electrocution and Fire Hazards
Hazard: The recalled power supplies are wired incorrectly, posing electrocution
and fire hazards.
Description: The recall involves RadioShack 13.8V DC Power Supplies, catalog
numbers 22-507 and 22-508 with date codes from 08A04 through 01A08. Date code
format is MMAYY where MM is the month and YY is the year. The catalog number and
date code are located on the back of the power supply. Power Supplies with a
green dot on the product and the product’s packaging have already been repaired
and are not included in the recall.

Redcat Racing Recalls Remote Controlled Toy Vehicles Due to Remote Control
Defect
Hazard: The remote control unit can lose its signal, causing the toy vehicle to

Hazard: The snaps on these garments can detach, posing a choking hazard to young
children.
Description: The recall involves two Taggies™ Sleep ‘n Play style infant
garments sold in purple or blue and white with style number listed in chart. The
infant garments are one piece footed coveralls with snaps down the front. These
garments were sold in sizes 0-3 months, 3-6 months, and 6-9 months. RN #27829 is

Children’s Jackets and Hoodies with Drawstrings Recalled by Coolibar Inc. Due
to Strangulation Hazard
Hazard: The garments have a drawstring through the hood, posing a strangulation
hazard to children. In February 1996, CPSC issued guidelines (pdf) to help
prevent children from strangling or getting entangled on the neck and waist
drawstrings in upper garments, such as jackets or sweatshirts.
Description: The recalled jackets and hoodies have a drawstring through the
hood. They were sold in children’s sizes 2 through 12. The jackets were sold in
the following colors: tan, stone, bayou blue, crisp blue, white, berry, and

Dollar Tree Recalls Glue Guns Due to Fire Hazard
Hazard: The recalled glue guns can short circuit, causing the gun to smoke. This
poses a fire hazard to consumers.
Description: This recall involves the “Crafters Square Hot Melt Mini Glue Gun”
with product number “939701”. The gun dispenses hot glue and is intended for
craft projects. The glue gun is black with an orange trigger and measures
approximately 4 1/2 inches from the back of the gun to the tip. It has a 44-inch
electrical cord attached. “Crafters Square” and “939701” are located on the
guns’ packaging. “DS-010 Glue Gun” is written in raised letters on the handle of
the gun.

A. O. Smith Recalls Water Pump Motors Due to Fire Hazard
Hazard: The water pumps with the recalled motors are labeled as containing a
one-horsepower motor, but actually contain a 3/4-horsepower motor. Under certain

Description: The recall involves motors included with lawn sprinkler pumps and

Counterfeit Circuit Breakers Recalled By Specialty Lamp International Due to
Fire Hazard
Hazard: The recalled circuit breakers labeled “Square D” have been determined by
Square D to be counterfeit and can fail to trip when they are overloaded, posing
a fire hazard to consumers.
Description: The counterfeit circuit breakers are black and are labeled as
Square D QO-series models 115, 120, 130, 215, 220, 230, 240, 250, 260 and 2020
and Square D QOB-series models 115, 120, 130, 220, 230, 250, 260 and 1515.
Actual Square D circuit breakers have (a) the amp rating written on the handle
in white paint on the front of the breaker (authentic Square D circuit breakers
manufactured prior to 2003 did not have white paint on the amperage numbers);
(b) the Square D insignia molded onto the breaker side, and; (c) a yellow
chromate mounting clip with half of the top of the clip visible. If your
breaker, labeled as Square D, does not match this description, it could be
counterfeit.

Stores Due to Fire, Burn Hazard
Hazard: On the recalled grills, the main burner does not fit correctly and does
not connect properly to the gas valve which could result in a propane leak. This
poses fire and burn hazards to consumers.
Description: This recall involves the Char-Broil gas grills with model number
463720108 and serial number ranging from 000001 to 000402. The gas grills have a
main burner with two-burner plates, a 170 square inch swing-away rack, two side
shelves and a condiment basket. Char-Broil’s logo is on the top of the lid of

valves installed in residential fireplaces made by twelve manufacturers under
the following brand names: Pacific Energy, Travis Industries, CFM Corporation,

Columbia, Twin Eagles, FDM, Inca Metals (Savannah Heating), Even Temp, and

Shims Bargain Recalls Pacifiers Due to Choking Hazard

Can Collapse
The entertainment centers can collapse if the back panel is not secure, posing a
risk of death or serious injury to consumers.
The recalled entertainment centers are black with two lower miter-framed doors,
two glass doors at the top, and CD storage racks. They measure about 54 inches
wide, 71 inches high, and 20 inches deep. They were sold under the Ridgewood/Charleswood

Old Navy Recalls Boys’ Jackets; Drawstring at Waist Poses Entrapment Hazard
Hazard: The jackets have a waist drawstring with a toggle that could become
snagged or caught in small spaces or doorways, which can pose an entrapment
hazard to children. In February 1996, CPSC issued guidelines (pdf) to help
prevent children from getting entangled on waist by drawstrings in upper
garments, such as jackets and sweatshirts.
Description: The performance fleece-lined shell jackets are hooded, zip in the
front, and have a drawstring at the waist. The jackets were sold in black or

Hazard: The plastic support brackets can break and cause the chairs to collapse,

Description: This recall involves the Lazy Lounger chair (Style # CEH00415),

Children’s Wooden Storage Rack Recalled By Jetmax International Following
Child’s Death
The storage rack can tip over, posing an entrapment and suffocation hazard to
young children.
CPSC has received one report of a death involving an 8-month-old boy who was
asphyxiated after he pulled on the storage rack and it fell over on him. The top
rail landed on the infant’s neck. No other incidents have been reported.
The storage rack is wooden with three levels and nine removable canvas totes.
There are wooden handles on each side of the rack. The boy’s storage rack has
natural color wood with red, yellow, green, and navy canvas totes. The girl’s
storage rack has white colored wood with pink, yellow, lime, and purple canvas
totes.
Wal-Mart sold the storage rack under the brand “Home Trend Kids 9 Canvas Bin
Boy’s and Girl’s Organizers.”
